A vital part of an effective Health and Safety program is the timely reporting and investigation of workplace incidents and/or accidents. The goal on an incident investigation is to identify the contributing factors and assist in the development of policies and/or procedures to prevent a re-occurrence. Ultimately, making the workplace safer for all Employees.
As part of their duties and responsibilities under the Occupational Health and Safety Act (OHSA) and Regulations, Employers have a legal duty to report workplace injuries and/or illnesses to both the Ministry of Labour, Training and Skills Development (MLTSD) and Workplace Safety and Insurance Board (WSIB). In co-operation with the Health and Safety Representative or JHSC, this requires the establishment of effective workplace policies and procedures for the thorough investigation of all workplace incidents and/or accidents. This includes the initial investigation, root cause analysis, reporting and recommendations. As well as, the development and delivery of Employee education and awareness supporting programs.

Areas of specialty include, human resources, accident re-construction, road safety, occupational Health and Safety, workplace Legislation, emergency rescue and management, workplace violence and mental health. Their extensive experience in incident and accident investigations includes root cause analysis, report writing, witness statements, court presentation and media relations. Investigation of workplace incidents of alleged employee misconduct and violation of company policy. This includes the review and interpretation of workplace policies, training and applicable Legislation. Depending on the circumstances, it may also include providing assistance with employee disciplinary procedures, including suspension, progressive termination or high-risk terminations.
In the event of an internal theft or allegation of property damage involving a worker or sub-contractor, experienced investigators will conduct a thorough investigation on behalf of the Employer. This may include an on-site investigation, evidence collection, witness interviews, surveillance and liaison with local police and/or MLTSD investigators.
Specifically focused on allegations of Workplace Violence and/or Harassment, these investigative services provide Employers with affordable investigative options. This includes
on-site investigations, witness statements and both Legislative and company policy review. As well as the development of Employee Safety Plans and disciplinary Corrective Action Plans (CAP).
Whether a non-union or unionized workplace, our Team of experienced Health and Safety
Co-ordinators have extensive experience with workplace mediation, employee counseling and collective bargaining. Depending on the circumstances, this may include the facilitation of
one-on-one interviews, group meetings or worker and/or supervisor mentoring.
As part of our client support services, the Critical Incident Response Team (C.I.R.T) is able to respond to a workplace or remote job-site on behalf of the Employer to conduct a thorough incident and/or accident investigation. Working in co-operation with the MLTSD, this includes initial investigation, scene security, media relations, evidence collection, root cause analysis, witness statements, report and recommendations, as well as court document preparation.
As part of the investigative process, our Team of highly-trained investigators has the knowledge and experience to prepare all required documentation for court purposes, including civil litigation, criminal prosecution, Provincial offences and/or employment tribunals . This includes testifying on behalf of an Employer, as well as pre-trial preparation of employees required to give testimony in a workplace legal matter.
